Insurance Information
Please call your insurance company to verify whether you have acupuncture benefits and if you can see an out of network practitioner. If you do, we will provide you with the appropriate insurance form (HECFA 1500) to submit to your insurance company.
If you're only allowed to see an in network practitioner, your insurance company will provide you with a list of in network providers.
We are not a provider for any specific insurance company.
If you have a Flex Account we will provide you with a 'Superbill' to submit to your company.
